Do you take insurance?
At this time, I do accept Blue Cross Blue Shield, Cigna PPO, and Amena for lactation services ONLY. Otherwise, I am private-pay only.
It is important to note that a superbill is available upon request and may help you secure reimbursement from your insurance provider for out-of-network costs (depending on your plan). Provision of a superbill does NOT guarantee reimbursement from your insurance provider.
Prior to starting services, it is the parent's responsibility to contact their insurance provider to secure any necessary pre-authorizations.
Payment for evaluation and treatment sessions are due at the beginning of each session.
